  1. Brief description of any problem your are experiencing (what were you doing, what page were you on, etc.),
  2. Browser type (Internet Explorer, Firefox, Safari, etc.),
  3. Browser version, and
  4. Computer platform (Windows XP, MacOSX, etc.)
